WASHINGTON, D.C. (21Alive) -- In a press conference today, President Barack Obama announced Veteran Affairs Secretary Eric Shinseki has resigned.

The resignation follows an investigation into mismanagement in the Department of Veteran Affairs health facilities. The President said that the problems were not just relegated to a few hospitals but were widespread throughout the country.

Some the charges against the VA included the falsification of records and problems with the quality of care provided at VA Hospitals. Last Friday, Shinseki offered an apology to veterans and called the problems "irresponsible and unacceptable."

An investigation into the Phoenix VA Hospital by the VA Office of Inspector General discovered that the average wait time for a primary care appointment was 115 days, but that the hospital reported their average as 24 days.
