FSSA: Possible Identities Distributed To Other Clients

INDIANAPOLIS, Ind. (www.TheIndyChannel.com) - Almost 4,000 clients of the Indiana Family and Social Services Administration might have had their social security numbers disclosed to other FSSA clients.

The FSSA announced Monday that an April computer programming error might have compromised the personal information of thousands of clients.

The agency, which serves 187,533 clients, said correspondence sent to clients between April 6 and May 21 might have included other clients' documents.

The following information might have been mistakenly distributed: name, address, case number, date of birth, gender, race, phone number, email address, types of benefits received, monthly benefit amount, employer information, financial information such as monthly income and expenses, bank balances and other assets, and certain medical information such as provider name, whether the client receives disability benefits and medical status or condition, and certain information about the client’s household members like name, gender and date of birth.

An additional 3,926 clients might have had their social security numbers revealed, the agency said.

The programming error was discovered May 10.
